Intake forms and why we love them
On your first appointment with one of the practitioners at Wintersweet Wellness Collective, you will be required to fill out an intake form. Many of us send a digital version of the intake form now, so you can fill it out up to 24 hours in advance of your appointment, saving you a bit of time and letting us review it before you arrive for your appointment. If you will be filling out a paper copy please be mindful that this can take some time, so it is recommended to arrive at your appointment 5 minutes early.
The intake form collects a bunch of information about you including your name, contact information and (very important) your health history. All of the information we collect about our clients is confidential and can only be shared with a third-party upon your signed approval. This is often the case when you need to communicate treatment information to other healthcare providers or need to provide documentation to your insurance company. If this is the case, they will have you sign a release form first, so you’ll be aware that it’s happening.
Your health history is really the important bit for us to review before your appointment starts. There can be many things that might not seem so important but will have a big impact on how we decide to go forward with your massage session. For example, if you have recently taken an anti-inflammatory drug you may not feel the same level of pain as you would normally feel. If this was the case, we would have to adjust the pressure down accordingly to avoid excess discomfort in the days following. If you’ve recently had surgery, dental work, or vaccinations we will have to work around that area differently. If you have any allergies or sensitivities we have the chance to review our products in advance so that we know things will be as safe for you as possible.
